Reef Kids Smile Plan

Ages Covered: 0–12 years
Annual Cost: $396
Annual Value: $789

For a 12-month period, members will receive:

• 2 child exams, cleanings, fluoride, with all necessary X-rays / panoramic
• 1 limited emergency exam with X-rays
• 20% off any additional treatment needs

Reef Teen Smiles Plan

Ages Covered: 13–20 years
Annual Cost: $444
Annual Value: $851

For a 12-month period, members will receive:

• 2 adult exams, cleanings, fluoride, with all necessary X-rays / panoramic
• 1 limited emergency exam with X-rays
• 20% off any additional treatment needs

Membership Plan Terms & Limitations

• Annual enrollment fees must be paid in full on or before the first visit.
• Enrollment fees are non-refundable and non-transferable.
• Benefits not used in an enrollment year cannot be carried over.
• Plan expires 365 days from the enrollment date.
• Membership plans are not dental insurance and cannot be combined with dental insurance.
• Plans cannot be applied to injuries involving lawsuits, motor vehicle accidents, medical insurance, workers comp, or homeowners insurance.
